GROUP 1 – Primary Identity Documents

  • A current Valid Passport (Any Nationality)
  • Biometric Residence Permit (UK)
  • Adoption Certificate (UK & Channel Islands)
  • Birth Certificate (UK, Channel Islands & UK authority overseas e.e embassy or HM Forces)
  • Current photocard driving licence – UK/Isle of Man/Channel Islands/other EU country (Full or Provisional)

GROUP 2a – Trusted Government Documents

  • Birth Certificate (UK & Channel Islands) issued more than 12 months after birth
  • Marriage/Civil Partnership Certificate (UK & Channel Islands)
  • Current photocard driving licence – All other countries (full or provisional) (Excluding UK/Isle of Man/Channel Islands & EU)
  • HM Forces ID Card (UK)
  • Fire Arms Licence (UK & Channel Islands)
  • Current paper version driving licence – UK/Isle of Man/Channel Islands & EU (Full or Provisional)

Group 2b – Financial and Social History Documents 

  • Work permit/VISA (residence permit) (UK)
  • Letter of sponsorship from future employment provider (Non-UK/Non-EEA only – Valid only for applicants residing outside of the UK at time of application)
  • EU National ID Card
  • Cards carrying the PASS accreditation logo (UK & Channel Islands only)
  • Letter from Head Teacher or College Principal of a UK Institution – Only to be used by 16 to 19 year olds in full time education if other documents cannot be provided.

The following documents must have been issued within the past 12 months

  • P45 certificate statement (UK & Channel Islands only)
  • Council Tax Statement (UK & Channel Islands only)
  • Mortgage Statement (UK or EEA) – (Non-EEA statements must not be accepted)
  • P60 certificate statement (UK & Channel Islands only)
  • Financial Statement (UK) – eg.g pension, endowment, ISA.

The Following documents must be less than 3 months old

  • Bank/Building Society Statement (UK & Channel Islands or EEA) (Non-EEA statements must not be accepted)
  • Credit Card Statement (UK or EEA) (Non-EEA statements must not be accepted)
  • Bank/Building Society Account Opening Conffirmation Letter (UK)

Correspondence Documents from:

  • Benefit Statement (UK) – e.g. Child Allowance, pension
  • Document from Central or Local Government, Government Agency, or local council document giving entitlement (UK & Channel Islands) – Document 1
  • Document from Central or Local Government, Government Agency, or local council document giving entitlement (UK & Channel Islands) – Document 2 (This must have been issued by a different body than document 1)

Utility Bills Such As:

  • Electricity Bill / Statement (UK)
  • Gas Bill / Statement (UK)
  • Water Bill / Statement (UK)
  • Telephone Bill / Statement (Not Mobile Telephone) (UK)

    At Achieve Sports we respect the privacy of the adults and children attending our clubs and the privacy of their parents or carers. The personal information that we collect about you and your child is used only to provide appropriate care for them, maintain our service to you, and communicate with you effectively. Our legal basis for processing the personal information relating to you and your child is so that we can fulfil our contract with you. Any information that you provide is kept secure. Data that is no longer required* is erased after you inform us that you or your child has ceased attending our club. We will use the contact details you give us to contact you via phone, text, email, social media, through our online booking system and post so that we can send you information about your child, our club and other relevant news, and also so that we can communicate with you regarding payment of our fees. We will only share personal information about you or your child with another organisation if we:

    • have a safeguarding concern about your child
    • are required to by government bodies or law enforcement agencies
    • engage a supplier to process data on our behalf
    • have obtained your prior permission.
    You have the right to ask to see the data that we have about yourself or your child, and to ask for any errors to be corrected. We will respond to all such requests within one month. You can also ask for the data to be deleted. If you have a complaint about how we have kept your information secure, or how we have responded to a request to access, update or erase your data, you can refer us to the Information Commissioner’s Office (ICO).
